Birth and Death Certificates

The Gasconade-Osage County Health Department is able to access records of births and deaths occurring throughout the state of Missouri.  We are able to issue certified copies after the state has placed that information on the computer. These can be obtained from any of our three locations.  The records are printed in the main office in Hermann, Missouri.

 

BIRTH-Certified birth certificates can be issued for persons born in Missouri since 1920.

DEATH-Certified death certificates can be issued for deaths occurring in Missouri since 1980.

 

Certificates that cannot be issued by this department must be obtained though the Missouri Department of Health in Jefferson City. For some purposes, such as genealogy or insurance, a certified copy of the original document is needed and can be requested from Jefferson City.

 

 

Questions

 

How are home births registered?

There is a special form that must be filed with the registrar in the county in which the birth occurred. The form must be completed correctly, signed by persons with knowledge of the birth, and be notarized before being submitted to the State. It is best to ask for assistance in filling out the form.

 

How can a birth or death record be corrected or changed?

The legal department of the Missouri Department of Health, Bureau of Vital Records must be contacted for assistance.

Cost per certified copy. Checks are accepted with identification. No credit or debit cards.

Birth Certificates --$15.00
Death Certificates --$13.00
$10.00 for each additional copy

Birth and death records are not public records. Only family members may be issued birth certificates. An application must be completed and signed. Photo ID is required when application is in person.

 

Certificates may be mailed when applications are received with a check or money order. Fax orders will not be accepted.
